Complete you Customer Agreement Form *REQUIRED*

Your customer agreement form is a document we require for every repair. This form allows us to operate the vehicle as necessary, as well as speak with your insurance company about all aspects of your claim.

If you have not already completed this form at one of our locations, or if your DHR Hail Advisor has not sent this form to you for signature, you will want to download, complete, and bring this form with you to your appointment. 

Make sure you and your car are ready for drop-off.  *HIGHLY RECOMMENDED*
